What is interprofessional care?

According to the World Health Organisation, interprofessional teams can provide a more comprehensive approach to preventing and managing chronic, complex conditions such as dementia, that may require a collaborative response (WHO, 2010; CIHC, 2010). Logan Healthy Living recognises the importance of interprofessional care in responding to the needs of the Logan community and brings together a growing number of disciplines.

Our diabetes education, exercise physiology, physiotherapy, dietetics and psychology practitioners offer multiple perspectives on your health and wellbeing. We offer a growing number of disciplines working together to explore new ways to meet your needs.
