Our free Type 2 Diabetes Lifestyle Management Program is completely bulk-billed and comes at no cost to participants 

 

Throughout the program, our allied health clinicians offer guidance, encouragement, support, and practical tools, to help participants build up their confidence in effectively managing their condition, work towards their goals, minimise complications and symptoms, and live a happier and healthier life. For participants who have a recent diagnosis (within the last six years), the aim can also be to support them in reaching remission and a symptom-free future.

Program Structure 

 

  Getting started: 

  • 1-on-1 welcome consult (1 hour)
  • 1-on-1 success planning consult (1 hour)

  The 8 week program

  • 8-week group program (comprised of weekly group movement and workshop/education sessions)

  Support after the 8 week program

  • 5 free recharge sessions (occurring 1 month, 3 months, 6 months, 9 months, and 12 months after the 8 week program)
  • Up to 5 free 1-on-1 consults per year, available as needed only if further support is needed 
    • If you have reached your limit of free consults within a year but would like to have more within the same year, you can request a care plan (GPCCMP referral) from your GP. Alternatively, if these options aren’t available to you, you can now book in for private one-on-one consultations. Click here for further information.

  1 year after completing the 8 week program, participants are eligible to recommence the program again.  

Getting Started
Welcome ConsultA 1 hour one-on-one consult with one of our allied health clinicians. In this consult the purpose is to get to know each other, and learn about you, what you want to achieve, what you feel could be holding you back, & how we can support you. We also introduce you to some of our team, and offer a site tour.
Success Plan ConsultThe success plan consult is another 1 hour one-on-one consult with one of our clinicians. In this consult, we work together to create a plan for you that combines the right kinds of practical support that fits you. No matter the change you’re looking for, or the guidance you’re after, our team is here to listen and offer the care and connections that can help you build a strong foundation for your long-term health and wellbeing.
8 week program
8 week group program

The 8-week program is comprised of weekly group sessions, facilitated by our allied health clinicians. Each week includes a group movement session, and a group workshop/education session. All sessions are 1 hour long. 

We recognise that navigating diabetes can feel overwhelming, so we have created a safe and relaxed environment where you can connect and share stories with others, learn at your own pace, exercise at your own level, explore your own health and wellbeing without any judgement what-so-ever, and feel supported & encouraged.

In these group sessions you become a part of a supportive community of like-minded people, who are participating in the program alongside you, and doing their best to support each other, learn with each other, and cheer each other on.

Post program support
5 Free Recharge SessionsAfter completing the 8-week group program, participants then receive 5 free recharge sessions. These occur at the intervals of 1 month, 3 months, 6 months, 9 months, and 12 months after completing the 8-week program. The first recharge session also includes a HbA1c test, and after the 12 month recharge session, participants are then eligible to recommence the program.
1-on-1 consults

Up to 5 free consults per year are available as needed, only if additional support is required. If you have reached your limit of these free consults within a year but would like to have more within the same year, you can request a care plan (GPCCMP referral) from your GP.

We understand that meaningful change happens in an environment where you feel understood and supported while being offered encouragement and guidance in a way that fits you and your life. 

At Logan Healthy Living, we aren’t here to hand you a generic list of ways you can exercise or eat a balanced diet, because what we learn from participants, is that what they really need and want is support. We hear your stories, that you’ve been told a million times what to do, or what not to do. You tell us that doesn’t work, and we get it.

While we hold knowledge and expertise as health clinicians, we know that the most valuable and important knowledge comes from you, because you are the expert on your own lived experience. You know yourself best, you know what has and hasn’t worked for you, what your journey has been, what matters to you, and what you need. Therefore, our role is to understand you and your experiences and listen to what it is that you need and want to achieve, so that we can uplift you with the kind of support and assistance that is shaped for you and what you’re looking for.

A GP referral letter is required to participate in our Lifestyle Management Program.
